Chandana Tripathy, MD, FACP

Medical Director MARC Primary Care
Dr Chandana Tripathy is a Professor/Clinical in the Primary Care Center and
Division of General and Hospital Medicine in the Department of Medicine at UT
Health San Antonio. Dr Tripathy is a Board Certified internist with over 15 years clincal expertise in Academic Primary Care setting. She currently serves as the Medical Director of the
Medical Arts and Research Center (MARC) Primary Care Clinic, the largest
primary care practice site in UT Health.
After completing her Internal Medicine residency from SUNY Buffalo in 2005, Dr
Tripathy joined UT Health Internal Medicine Clinic at the Texas Diabetes
Institute (TDI). During her term at TDI Dr Tripathy worked with an
inter-professional team and cared for a medically complex population with
Diabetes and associated chronic health conditions and challenging Social
determinants of health. This experience prepared her to lead the largest Primary
care practice in UT health. She has successfully led her practice to achieve
Patient Centered Medical Home (PCMH) accreditation. Completing the UT System‘s
Clinical Safety and Effectiveness Course equipped her to lead several practice
improvement projects and redesign clinic workflow to enhance efficiency in her
practice. Completing the TEAM STEPPS Master Trainer Course and training
health care providers and staff in my clinic resulted in improved communication
and patient safety. She is committed to improving Population Health and Value Based Care delivery performance such as DSRIP - Texas
Medicaid delivery system reform and ACO-Medicare Shared Savings Program.
As a Clinical Faculty and Clinician leader in the Long School of Medicine her
professional goals focus on providing compassionate, safe, and evidence-based
care to patients, improving the health of the South Texas community, and
teaching and mentoring the next generation of physicians. She is committed to
Primary Care practice transformation to increase patient access, decrease
health disparities among patients in San Antonio and lowering health care
spending by keeping people healthy and out of the hospital.
