Polly Hitchcock Noel, PhD is a Professor/Research in the Department of Family and Community Medicine. Dr. Noel is a clinical health psychologist with over 20 years of experience as a health services researcher. Her research has focused primarily on evaluating and improving quality of chronic illness care, especially for individuals with chronic conditions that are under-recognized or sub-optimally managed in the primary care setting.

Funding Agency Veterans Administration Title VA Vascular Injury Study (VAVIS): VA-DOD extremity injury outcomes collaboration Status Active Period 4/2015 - 9/2020 Role Co-Investigator Grant Detail This grant will define long-term outcomes of extremity vascular repairs, which are currently not available in the civilian or military trauma literature in regards to limb function and repair patency. Furthermore, monitoring of the repair with duplex ultrasound and drug therapies such as antiplatelet agents to improve patency rates are based upon studies in patients with atherosclerosis as the etiology of the vascular repair rather than traumatic injury. To better understand the outcomes and needs of this group of Veterans, the following 3 aims will be performed: 1) Compare injury, demographic and geospatial characteristics of patients with initial in-theatre limb salvage (IILS) and identify late vascular surgery related limb complications and health care utilization in Veterans receiving VA vs. non-VA care. 2) Characterize the preventive services received by individuals with vascular repair and related outcomes. 3) Describe patient-reported functional outcomes in Veterans with traumatic vascular limb injuries. Specific objectives are to: 1) Elucidate how sensemaking unfolds across professional cultures and organizational units to reduce readmissions, the origins of which cannot easily be attributed to any one single organizational or patient level failure; and 2) Build an agent-based model based on results from the qualitative data to gain further insights into the interdependencies and relative effects of the various agents and their actions on readmission rates. Status Active Period 1/2017 - 12/2017 Role Principal Investigator Grant Detail This pilot project will assess the feasibility, acceptability, and preferred strategies for implementing a referral and feedback system between primary care providers and local multipurpose senior centers operated by the City of San Antonio. Findings will inform the development of future implementation research to establish and enhance primary care-senior center partnerships to help prevent and control diabetes in socioeconomically vulnerable, older primary care patients. -
